Lab Reviews
Please be sure to schedule a lab review at least 1 week after your lab draw appointment. No questions about labs will be answered unless an appointment is scheduled to review them with the provider.
Pre Op Clearance
Please ensure you have provided the information needed for your surgery. This may include a form or just faxed instructions by the surgeon’s office explaining what is needed to clear the patient for surgery.

Annual Wellness
Yearly wellness appointments often include routine bloodwork. Please prepare by fasting, eating nothing after midnight, and drinking plenty of water. You may also drink black coffee. Please also arrive 30 minutes early to complete any paperwork unless you have filled them out and have them readily available for your appointment, which is our preferred method.
Sick Visits
Acute sick visits that include any viral symptoms, as well as pains, allergies,

This is a routine appointment that varies by age. Please prepare for this appointment by arriving 15 minutes early to fill out the well-child form.
Women’s Health
Yearly gynecological care is provided. This includes pap smears. Orders for Mammograms, ultrasounds, and bloodwork if needed.

Quick visits that are done by medical staff. This can be scheduled blood draws or injections.
New Patient Appointment
We are accepting new patients and would prefer that all paperwork is completed before hand if at all possible. You may stop by the office to pick up paperwork or fill it out while in the office. You may also download, print, and fill out the paperwork and have it readily available for your appointment. We ask that new patients arrive at least 15 minutes early. We will need copies of ID’s and Insurance cards to include secondary insurances and drug cards if applicable. Also, if expecting blood work, please prepare by fasting, having no food or beverages aside from plain water after midnight. Please also be kind and let us know at least 24-hours in advance if you need to cancel or reschedule your visit.
